Community health survey to be conducted  

LYONS - The Wayne County Public Health Department is conducting a survey regarding the health needs and status of county residents in conjunction with the S2AY Rural Health Network. The survey is designed to collect information on peoples personal experiences with the health care system and what they believe are the major health concerns in the county.

The survey is available on the S2AY Rural Health Networks website, www.s2aynetwork.org, and at www.co.wayne.ny.us. Residents may also request a copy of the survey by calling 946-5749.

The survey is part of an overall community health assessment process called Mobilizing for Action through Partnership and Planning, which looks at the public health system; community health status; and local, state and national factors that influence health. The survey will be distributed through March 13; survey results will be tabulated and shared by the fall. The summary of the survey will be included in Wayne County's Community Health Assessment, which will be completed by September. For further information, call 946-5749.
